Add Medication History

Add current medications to a patient's Health Summary that your patient is taking but which were prescribed elsewhere, such as in hospital or at another service.

Before you begin

Permissions:
  • Update Medications

You cannot record a medication with a date before a patient's birth or after a patient's date of death.

The medication history of a patient is not shared with My Health Record or MeHR.

About this task

Note:
You cannot print or issue repeats for medications recorded as medication history. These medications must first be prescribed.

Procedure

To add medication history for a patient:
  1. In the patient's Health Summary iconHealth Summary, click + Medication.
  2. In the Medication History iconMedication panel on the right, in the Search iconSearch field, enter at least the first two characters of the medication.
    Tip:
    Alternatively, select a medication from your favourites list, or go to the Recipes tab an extemporaneous preparation recipe, click + Recipe. For more information about creating drug recipes, see Create Medication Recipes.
  3. Select the medication from the list.

  4. Complete as much information as possible, including at least:
    • In the Directions field, include the patient's directions.
    • In the Advised or prescribed externally by field, enter where the information about the medication is coming from. For example, the patient or a discharge summary.
  5. Click Save.

Results

The medication is immediately listed in the patient's Health Summary iconHealth Summary without a Last Script date.

A new medication history is added to the Consult iconConsult > Consult Actions > Other Actions section. When you click on the medication, its information is displayed in the medication history panelMedication panel.

Tip:
If any medication history is added during a patient consultation, it is displayed in the existing consultation. Otherwise, a new consultation is created automatically to record the changes.

What to do next

In the Health Summary iconHealth Summary section:
  • If required, to edit a medication history item, click the Vertical ellipsis iconVertical ellipsis and select Edit or alternatively, select the required medication history and in the Medication History iconMedication panel, click Edit at the bottom. For more information about editing medication history, see Edit Medications.
  • If required, to cease a medication history item, click the Vertical ellipsis iconVertical ellipsis and select Cease or alternatively, select the required medication history and in the Medication History iconMedication panel, click Cease at the bottom. For more information about ceasing medication history, see Cease Medications.
  • If you want the patient to continue taking this medication, prescribe it. You can prescribe the medication by either:
    • Hover your mouse over Rx icon in Health Summary and click Quick Prescribe.
    • Select the medication history medication to display its details in the Medication History iconMedication panel and click Prescribe.

To update the newly added medication history details during a patient consultation, in the Consult iconConsult > Consult Actions > Other Actions section:
  1. If you have updated the details multiple times, select the most recent recorded medication item. Otherwise, select the required medication item.
  2. In the medication history panelMedication panel, click Edit.
  3. Update the required medication history details.
  4. Click Save.
