Use the Chronic Disease Management (CDM) feature to add a GP Mental Health Treatment Plan for patients.

- Health Summary
  Information entered by a practitioner during a consult is added to a patient's Health Summary.
- Consultations
  Record details of a clinical consultation in the Consult panel of the patient's record.
- Patient Timeline
  A summary of the patient's clinical history is displayed in the Patient Timeline.
- Medications
  Helix uses the MDref medications database provided by AusDI.
- Immunisations
  Before you administer an immunisation to a patient, check the immunisations that have already been administered on the AIR and the immunisations due.
- Allergies
  A patient's drug allergies can be added to their record.

  - Care Plans in the Timeline
    When you exit a consult, all care plans, referral letters, and EDC forms are removed from the Actions section, and added to a new consult entry in the patient's Timeline.
  - Cycle of Care
    Use the Chronic Disease Management (CDM) feature to define the cycle of care for patients with diabetes or asthma.
  - GP Mental Health Treatment Plan
    Use the Chronic Disease Management (CDM) feature to add a GP Mental Health Treatment Plan for patients.
- History & Examinations
  Record details of a patient's clinical history in the History section. Record details of today's examination of a patient in the Examination section.
- Measurements and Assessments
  Record metabolic, pathology and ECG measurements and other assessments in the patient's record as required.
- Recalls
  A Recall is a type of flag recorded against a patient's record that informs practitioners and practice staff of when the patient is due to return to the practice.
- Nursing Requests
  During a consult, add a request for a nurse to complete a nursing action.
- Obstetrics
  Record obstetrics information on the Obstetrics panel.
- Investigations
  Investigations can be requested by a practitioner from within a patient's clinical record.
- Tasks
  The tasks module allows users to send messages (tasks) to other users throughout the medical centre.
- My Health Record
  Securely upload a patient's health data to their My Health Record.

GP Mental Health Treatment Plan

Use the Chronic Disease Management (CDM) feature to add a GP Mental Health Treatment Plan for patients.

Before you begin

Permissions:

View Mental Health Care Plans
View Care Plans
Update Care Plans
View Consults
Update Consults

Procedure

To add a GP Mental Health Treatment Plan:

From the Patient Search or Patient Queue, select the Consult panel.
In the Consult panel, select More > CDM.
In the CDM panel, select GP Mental Health Treatment Plan.
If you have the Create Care Plan on behalf of HCP permission and are creating the care plan on behalf of a health care professional, select their name from the HCP list and click OK.
In the GP Mental Health Treatment Plan, the existing authority information, Conditions, Health Summary, and biometrics for the patient are displayed on the left-hand side.
- Authority Information - select options to include them in the plan.
- Assessments - if no information is shown in the Assessments section, access the Assessments panel, and complete the listed assessment.
- Medications - select medications to include them in the plan.
- Condtions - select condtions to include them in the plan.
- Health Summary - the patient's health summary from their patient record.
- Measurements - to update this information, click Update.
On the right-hand side, enter information for each section:
- Mental Health History/Treatment
- Mental Status Examination
- Risk Assessment
- Mental Health Treatment Plan
- Initial Action Plan
A recall is set for 6 months from the current date. Edit the date if desired.
If the treatment plan is not yet complete, click Save as Draft.

A draft of the GP Mental Health Treatment Plan is saved in the GP Mental Health Treatment Plan section.

To continue working on the GP Mental Health Treatment Plan, click Resume.
When you have completed the form, click Save and Print.

Results

The GP Mental Health Treatment Plan is saved to the Actions section of the Consult panel and printed.

When the consult ends, the plan is added to the Patient Timeline icon Patient Timeline. For more information, see Care Plans in the Timeline.

What to do next

Later, when you are ready to review the plan, click Review. The form opens again, but the fields available for input are different because this is a review of the initial assessment.
