Cycle of Care
Use the Chronic Disease Management (CDM) feature to define the cycle of care for patients with diabetes or asthma.

- Health Summary
  Information entered by a practitioner during a consult is added to a patient's Health Summary.
- Consultations
  Record details of a clinical consultation in the Consult panel of the patient's record.
- Patient Timeline
  A summary of the patient's clinical history is displayed in the Patient Timeline.
- Medications
  Helix uses the MDref medications database provided by AusDI.
- Immunisations
  Before you administer an immunisation to a patient, check the immunisations that have already been administered on the AIR and the immunisations due.
- Allergies
  A patient's drug allergies can be added to their record.

    When you exit a consult, all care plans, referral letters, and EDC forms are removed from the Actions section, and added to a new consult entry in the patient's Timeline.

  - GP Mental Health Treatment Plan
    Use the Chronic Disease Management (CDM) feature to add a GP Mental Health Treatment Plan for patients.
- History & Examinations
  Record details of a patient's clinical history in the History section. Record details of today's examination of a patient in the Examination section.
- Measurements and Assessments
  Record metabolic, pathology and ECG measurements and other assessments in the patient's record as required.
- Recalls
  A Recall is a type of flag recorded against a patient's record that informs practitioners and practice staff of when the patient is due to return to the practice.
- Nursing Requests
  During a consult, add a request for a nurse to complete a nursing action.
- Obstetrics
  Record obstetrics information on the Obstetrics panel.
- Investigations
  Investigations can be requested by a practitioner from within a patient's clinical record.
- Tasks
  The tasks module allows users to send messages (tasks) to other users throughout the medical centre.
- My Health Record
  Securely upload a patient's health data to their My Health Record.

Cycle of Care

Use the Chronic Disease Management (CDM) feature to define the cycle of care for patients with diabetes or asthma.

Procedure

To add a cycle of care:

From the Patient Search or Patient Queue, select the Consult panel.
In the Consult panel, select More > CDM.
From the Cycle of Care section of the CDM panel, select Asthma or Diabetes. In our example, we have selected Asthma.
A list of care steps related to the condition is displayed. When a step has been completed, tick the box beside it.
A recall is set for six months from the current date. To edit the recall date, click the calendar and select a new date.
Click Save as Draft.

Results

A draft of the cycle of care plan is saved in the Current Cycle of Care section.

What to do next

To update the cycle of care:

To continue working on the cycle of care, in the CDM, click Resume.
To delete the cycle of care, click Delete .

When all items in the cycle of care have been ticked and the cycle of care is complete, click Complete. In the CDM panel, the status of the cycle of care is changed Completed .
